Owner Surrender

If you need to surrender an animal to the shelter, please download and fill out the form below and bring it with you when you bring in the animal.  

We only accept surrenders during our business hours.  Our premises are under video surveillance at all times.

Our hours of operation are: 

• Mondays: 9:00-2:00 Central 
• Tuesdays: 9:00-2:00 Central
• Wednesdays: CLOSED
• Thursdays: 9:00-2:00 Central
• Fridays: 9:00-2:00 Central
• Saturdays: 9:00-1:00 Central
• Sundays: CLOSED
